In order for campers to attend Bar-T, we need to have the required health forms on file.
All Campers
Camper Health Forms can be completed online within your account at registration.bar-t.com. Please email us at help@bar-t.com if you have any trouble with the forms.
Camper Medication Forms
Medication Administration Authorization Forms – Must be turned in if your camper may need to take any medication(s) while at camp. New this year: Per state licensing requirements, this form must be signed by a doctor.
