Waivers & Permission Slips
Some of our camps require additional permission slips for campers to be able to participate in certain activities at camp. Please check the sections below to see if any additional forms are required for your child’s camp.
Ranch Campers (including Specialty Camps)
- A Camper Health Form must be completed online for each camper. It can be completed online at http://registration.bar-t.com.
- Swim Lessons: Would your camper like to become a more proficient swimmer this summer? Sign them up for optional swim lessons before or after camp. Please note that transportation is not provided for campers taking swim lessons, and checks should be made payable to the guard giving the lesson. First come first served. To schedule your swim lesson, please submit this form or email us at ranchpool@bar-t.com.
- Ranch Specialty Camp Forms: These forms must be signed and returned in order for campers to participate in their respective specialty camp. If there is not a permission slip listed for the camp that your child is signed up for, you do not need to take any additional action.
Mountainside Campers (including Specialty Camps)

How to Submit Forms
All paper forms can be returned to the Director at Parent Orientation or to the Bar-T Team when your drop-off your camper for the first day.
